Established that childhood neglect impairs cognitive and brain development; debate concerns the deprivation/threat distinction and reversibility, not whether neglect harms.
Anchored by the Bucharest Early Intervention Project (the only RCT of foster care for institutionally deprived children; Sheridan et al., 2022) showing causal effects of deprivation on cortical development, plus a large prospective birth cohort linking neglect to cognitive deficits from age 16 to 50 independent of mental health (Geoffroy et al., 2016) and converging neurodevelopmental reviews. Effects are strongest for early, chronic, severe deprivation; the deprivation (cognitive) and threat (abuse) dimensions have partly dissociable neural signatures.

Experimental
Using the only RCT of foster care for institutionally deprived children, demonstrates a causal link between early psychosocial deprivation and altered cortical development (prefrontal cortex, white-matter tracts); placement into family care mitigated the effects.

Outside the institutional-deprivation RCTs, most studies are observational and confounded with poverty and co-occurring adversity; "neglect" is heterogeneous and inconsistently measured; reversibility and sensitive-period boundaries are not fully resolved; and brain-structure findings do not map one-to-one onto individual impairment.

Neglect, the most common form of maltreatment, has severe short- and long-term cognitive, social-emotional, and behavioral effects; relative to physically abused children, neglected children show more severe cognitive/academic deficits and social withdrawal; early neglect is particularly detrimental.
