Established that intensive, structured, family-centered early intervention improves outcomes for young children with delays or disabilities; debate concerns magnitude, domain-specificity, and timing.
Supported by meta-analyses dating to Shonkoff & Hauser-Cram (1987) and the Ramey synthesis, RCTs of routines-based/family-centered intervention (Hwang 2013), and developmental-neuroscience framing of plasticity (Nelson 2023). Gains are largest for intensive, high-quality, parent-and-child-targeted programs; for autism specifically, very-early effects on neurodevelopmental outcomes by age 3 are modest and uncertain.

Intensive, high-quality, ecologically pervasive early interventions yield significant cognitive, academic, and social gains, whereas fragmented, weak efforts are unlikely to succeed.
