There is settled scientific agreement that early-childhood lead exposure impairs cognitive and behavioral development, and that no safe blood-lead level has been established in children — the CDC, AAP, and WHO all set a reference value rather than a safe threshold. The relationship is graded (more lead, more harm), holds across countries and decades, and is supported not only by observational cohorts but by the natural experiment of the leaded-gasoline phase-out, so it reflects a causal effect and not mere correlation. What remains debated is only the precise size of the effect at the very lowest exposures, not whether the harm is real.
Population-level. The dose-response is supralinear — the greatest marginal harm occurs at the lowest exposures. Causal direction is supported beyond observational association by quasi-experimental evidence (e.g., the phase-out of leaded gasoline). Residual confounding by maternal IQ and socioeconomic status persists at the very lowest measured exposures (<5 ug/dL), where some cohorts attenuate after adjustment.

Effect magnitude at the lowest exposures is debated and is where residual socioeconomic confounding is hardest to exclude; some cohorts attenuate after adjustment below 5 ug/dL. The harm is population-level and does not establish that lead is the sole or primary driver of any individual child's outcome. Magnitudes observed at high historical exposures should not be transferred to today's lower population exposures. Remediation/clinical-reversal evidence is separate and limited.
