The operative claimParental substance use disorder is a moderate risk factor for child maltreatment and for worse developmental, behavioral, and mental-health outcomes, operating largely through impaired, inconsistent, or disengaged caregiving; the risk is graded by severity of use and substance type, is confounded by co-occurring adversity, and substance use alone does not establish maltreatment or parental unfitness.
Established that parental substance use disorder is a risk factor for maltreatment and worse child outcomes; the agreed nuance is that the average effect is small, graded by severity, and confounded.
Supported by a multilevel meta-analysis of 56 longitudinal studies (small average effect r = .15, larger for drug use r = .25; Kuppens et al., 2020) and converging systematic reviews on opioid and nondependent use. A national-sample study shows the risk is graded: only past-year substance use DISORDER (not lower-level use) predicted higher neglect frequency (Kepple, 2018). Effects are confounded with poverty, mental illness, and family instability; family-centered treatment improves outcomes.
