The operative claimYouth in the juvenile justice system carry a far higher burden of trauma exposure and trauma-related psychiatric disorder than adolescents in the community: roughly nine in ten detained youth report traumatic exposure — typically multiple, beginning early in childhood — and PTSD prevalence (about 9–24%, higher in girls) is several times community rates.
It is settled science that youth who reach the juvenile justice system have experienced trauma at extraordinarily high rates — most of them multiple traumas beginning in early childhood — and meet criteria for PTSD, depression, and related disorders at several times community rates, with girls carrying roughly twice the trauma-related burden of boys. Disagreement in the field concerns how systems should respond, not whether the burden exists.
Applies to youth at all levels of juvenile-justice processing (intake, probation/community supervision, detention, secure post-adjudication), with the heaviest documented burden in detained samples. Prevalence estimates vary with instrument, informant, and setting; girls consistently show roughly double the rates of PTSD and depression of boys.

92.5% of detainees had experienced one or more traumas (mean 14.6 incidents; median 6); 93.2% of males and 84.0% of females reported at least one traumatic experience. 11.2% met criteria for PTSD in the past year; more than half of those with PTSD reported witnessing violence as the precipitating trauma. Trauma and PTSD are far more prevalent among juvenile detainees than in community samples.

Trauma exposure typically began early and persisted: 62% were first exposed within the first five years of life, and about one-third were exposed to multiple or co-occurring trauma types each year into adolescence. 23.6% met criteria for PTSD, 66.1% were in the clinical range for externalizing problems, and 45.5% for internalizing problems; up to 90% of justice-involved youth report some traumatic exposure.

Rates of every type of traumatic exposure and of PTSD were clearly elevated at ALL levels of juvenile-justice processing, not only in deep-end placements. Males more often reported assaultive violence; females were significantly more likely to report forced sexual activity. Traumatic victimization and externalizing behavior were interconnected, supporting treatment approaches that address their co-occurrence.

Prevalence estimates depend on instrument, informant, and setting and vary considerably across studies; most designs are cross-sectional, so the temporal ordering of exposure and justice involvement is not established by these data (though early onset ages make reverse causation implausible for much exposure). Self-report may over- or under-count. Comparison "community rates" come from different samples and instruments. None of this evidence establishes that trauma causes offending in an individual case.
