Childhood neglect — the chronic absence of expected cognitive, linguistic, social, and caregiving input — is associated with substantial, often lasting deficits in cognitive ability, executive function, and brain development; the deprivation pathway is distinct from the threat pathway of abuse, and randomized evidence from early-deprivation studies supports a causal contribution.

Childhood sexual abuse is associated with a broad, dose-responsive elevation in long-term psychiatric and physical-health risk — depression, anxiety, PTSD, suicidality, and substance dependence among them — and the strongest designs (umbrella reviews, longitudinal cohorts, discordant-twin studies) indicate the association is at least partly causal and not merely a product of shared family confounding; nonetheless many survivors show resilience and no measurable lasting impairment.

Children's exposure to intimate partner violence (IPV) between caregivers is prospectively associated with elevated internalizing and externalizing problems and other adjustment difficulties; the association is robust across many studies and strengthens over time, but it is modest in magnitude, varies widely across children, and operates substantially through co-occurring maltreatment, impaired caregiving, and maternal mental health rather than as a uniform, direct effect of witnessing alone.

For adolescents in out-of-home care, congregate (group/residential) placement is associated with worse outcomes than family-based foster care — including roughly 2.5 times the risk of delinquency in matched samples, worse behavioral outcomes meta-analytically, and more criminal referrals than treatment foster care in a randomized trial — though selection of higher-risk youth into congregate settings accounts for part of the difference, and the association is weakest for youth who were already delinquent.

Household food insecurity is associated with poorer academic and cognitive functioning and increased behavioral and mental-health risk in children; severity, persistence, and transitions in food security may matter.

Whether placing a maltreated child in foster care produces better or worse outcomes than leaving the child at home is genuinely unresolved: rigorous quasi-experiments using the same identification strategy reach opposite conclusions, effects apply only to "marginal" cases at the placement threshold, and the science supports neither a presumption for nor against removal.

For young children deprived of parental care, placement into stable, high-quality family (foster) care rather than institutional care produces lasting gains in cognitive ability, attachment security, and mental health; benefits are larger with earlier placement and greater placement stability, although some deficits persist.

Placing siblings together in out-of-home care is best practice and federal policy and is associated with greater placement stability, but the evidence that co-placement improves child well-being is mixed and confounded; the benefit appears to track sibling-relationship quality more than co-placement itself, and separation can be appropriate when one sibling endangers another.

Children placed with kin (relatives) in foster care generally show greater placement stability and fewer behavioral and mental-health problems than children in non-relative foster care; but much of the behavioral advantage reflects selection, and rigorous designs that adjust for it find the stability benefit more robust than the behavioral and academic benefits, which are mixed.

Parental incarceration is associated with worse child outcomes — particularly antisocial behavior and, into young adulthood, anxiety, substance-use disorder, and reduced educational attainment — but the most rigorous studies, which control for pre-existing antisocial behavior and shared genetic/family confounding, find the independent effect is smaller and uneven across domains; paternal-incarceration effects are better supported than maternal, and much of the raw association reflects selection.

Parental substance use disorder is a moderate risk factor for child maltreatment and for worse developmental, behavioral, and mental-health outcomes, operating largely through impaired, inconsistent, or disengaged caregiving; the risk is graded by severity of use and substance type, is confounded by co-occurring adversity, and substance use alone does not establish maltreatment or parental unfitness.

Among children in out-of-home care, placement instability (frequent placement changes) is associated with worse behavioral and mental-health outcomes — especially externalizing problems and PTSD symptoms — and propensity-adjusted work indicates an effect of instability beyond children's baseline problems; the relationship is partly bidirectional, since behavioral difficulties also drive disruptions.

Maternal smoking during pregnancy is associated with some adverse neurodevelopmental outcomes, including developmental coordination, small average IQ differences, ADHD diagnoses, and selected brain or behavioral measures, but causality is outcome-specific; genetically sensitive research does not support a clear causal prenatal-smoking-to-ADHD relationship.

Children in foster care are prescribed psychotropic medications — antipsychotics and multi-drug regimens in particular — at rates several times those of otherwise-comparable Medicaid-enrolled children, including the high-risk practices (concomitant antipsychotics, four-plus-drug polypharmacy) with the least pediatric evidence; state oversight programs measurably reduce this prescribing, which is why federal law requires psychotropic monitoring protocols for children in care.

Black children are substantially overrepresented in the U.S. child-welfare system — in foster care at roughly twice their share of the child population — but the cause is genuinely contested: quasi-random-assignment evidence from one large system found Black children 55% more likely to be placed than white children with identical potential for future maltreatment, while national and state benchmark studies find reporting disparities no larger than underlying poverty-and-harm disparities and equal-or-lower substantiation and placement odds after a report, locating the disparity upstream in structural socioeconomic conditions.

Randomized human evidence shows that severe early deprivation of stable, responsive caregiving can cause lasting cognitive and psychiatric harm, and that early placement into stable, high-quality family care can substantially improve outcomes.
